New Students

  • Form 138 (Original SHS Card)
  • Certificate of Good Moral (original copy)
  • PSA (Formerly known as NSO) Birth Certificate (photocopy)
  • Transcript of Records
  • Certificate of Transfer credentials
  • 7 pcs. 1×1 ID picture (in formal attire, white background)
  • 6 pcs. Mailing envelope w/ stamp
  • NCAE Result (original copy)
  • Form 137 Original Permanent Record

Transferees

  • Honorable Dismissal Transcript of Records (T.O.R.)
  • 7 pcs. Latest (1×1) colored picture.
  • Certification of Good Moral Character
  • Photocopy of NSO/PSA Birth Certificate
  • 6 pcs. Mailing envelope, self-addressed and stamped

Maritime Students

Aside from the requirements above Maritime applicants are required to undergo:

  • Physical Examination and Color Blindness Test
  • Interview with Maritime Department Head

Caregiver and other Technical Education Courses

For the Caregiver course and other short courses under TESDA, the requirements are as follows:

  • High School Graduate form (Form 137) or transcript of records (3 copies)
  • 2 pcs 2×2 id picture and 4 pcs 1×1 id picture
  • Marriage contract, if married female (3 copies)
  • Long brown envelope 1 pc

All college and modular courses are only available at the main campus

Pre-Registration

If you wish to register ahead of time it is very easy. Simply download the pre-registration form and submit to the registrar or go to the registrar and fill it up there. You may also fill up the form and email: fcatregistrar@yahoo.com
